Large Animal Practice Sterilization: What Changes on a Farm-Call Workflow

A practice that runs farm calls works out of two places at once. The base unit stores equipment, supplies and records; the barn, crush or paddock is where surgery happens. Ontario's facility standards follow that split, with a section written for the base unit of a mobile practice and a separate standard for large animal ambulatory surgery.

What does a farm call change about sterilization?

The method does not change; the sequence does. A recognized method of sterilization must be employed, the system has to be effective in the sterilization of instruments and equipment, and a steam sterilizer large enough for the surgical packs of the expected case load has to be present. A barn has no autoclave, so packs are processed, packaged and labelled at the base unit before the truck leaves. The ambulatory standard asks for aseptic technique as appropriate and practical for the procedure and conditions.

Is cold sterile acceptable for field surgery?

Only for the items the standards name. Cold sterilization is listed for material and instruments that cannot be steam sterilized, such as instruments with lenses (endoscopes and arthroscopes) and anesthetic equipment. The standard is unambiguous the other way: cold sterile is not an acceptable sterilization method for surgical instruments to be used for major surgery.

What has to travel in the truck for a farm call?

The ambulatory surgery standard asks for:

  • Separate sterilized surgical packs for each major surgical procedure.
  • Sterile single-use surgical gloves for all surgeries, plus sterile instruments, towels and drapes for each procedure.
  • Hand and arm scrub supplies including a surgical scrub agent; a recognized brushless system is acceptable.
  • Footwear that can be easily disinfected or disposable footwear, and washable or disposable coveralls, cleaned immediately after use.
  • Contained waste: a 30-gallon red biohazard bag and heavy-duty nitrile gloves carried for the visit rather than borrowed from the clinic bin.

Which records does a mobile practice keep on a sterilized pack?

Every item sterilized in the facility must display the date of sterilization and the name or initials of the team member who carried out the sterilization, and surgical packs are initialled and dated by the person who processed them. Autoclave efficiency must be routinely monitored using standard sterilization indicators, because pressure and temperature gauges may be inaccurate. For a mobile practice the workable version is one line per cycle in a log that travels with the kit. How is farm-call waste classified in Ontario?

Saturation decides the stream, not the location. Under C-4, animal blood waste means waste related to an animal treated for an infectious substance that is liquid or semi-liquid blood, items saturated with it, or body fluids visibly containing animal blood; urine, faeces and milk are excluded unless visibly bloody. Containers must be capable of withstanding the weight of the waste without tearing, cracking or crushing.
