Outbreak and incident investigation. If a patient develops a post-treatment infection or a positive spore test surfaces, the log is how the clinic can reconstruct which instruments were in which cycle, whether the parameters were met, and what corrective action was taken. Without records, every instrument processed in the suspect period is unverifiable.

What a Compliant Sterilization Log Records
There is no single mandatory national template, but provincial regulators and the technical literature converge on the same fields. A compliant entry captures:
- Date and time of the cycle.
- Sterilizer identification — unit number or name, if the clinic has more than one.
- Cycle type — wrapped, unwrapped, liquid, or implant cycle, and the selected parameters (temperature, time, pressure).
- Load contents — a summary of what was processed (e.g., "20 dental hand instruments, 4 surgical kits").
- Chemical indicator results — external indicator (tape) and internal indicator (pouch) pass/fail per pack.
- Biological indicator result — spore test lot number, incubator temperature, and pass/fail, when a BI was included.
- Mechanical readings — recorded temperature and pressure from the sterilizer display or chart recorder.
- Operator initials — who loaded, ran, and released the load.
- Corrective actions — anything unusual, plus the follow-up taken.
Fill in the entry at the time the cycle runs, not at the end of the week. Retroactive entries are a credibility problem during any investigation.

How to Use the Log
Set a simple routine so the log becomes automatic:
- Keep the binder in the sterilization room, next to the autoclave — not in the office.
- Complete each entry at cycle end, while the display readings are still in front of you.
- Record chemical indicator results per pack. For the log, note the number of packs and whether all internal indicators changed correctly.
- Log the weekly spore test with the BI lot number, and update the result the next day after incubation.
- Add a corrective action note for any abnormal reading — and follow through.

Digital vs. Paper Autoclave Log Sheets
Clinics ask whether a printed autoclave log sheet is still the right approach, or whether a spreadsheet or app is better. Both work; the choice depends on your team and your risk profile.
Paper advantages. A paper binder is simple, visible, and tamper-evident. Entries are made by hand at the time of the cycle, and any alteration is obvious. It works during power outages, requires no training, and costs nothing to run. For a one- or two-autoclave clinic, paper is usually the lower-friction option.
Digital advantages. A spreadsheet or compliance app can flag missed weekly spore tests, auto-calculate date ranges, and store sterilizer maintenance records in the same file. Digital logs make monthly review faster and allow you to export a clean report for an inspection. The trade-offs are the risk of silent data loss (no backup, laptop failure), the need for staff discipline to open the file at cycle end, and the question of whether the file was edited after the fact.
If you go digital, keep the same discipline as paper: make the entry at cycle end, use a locked or audit-trail format, back up regularly, and keep a paper or PDF archive of completed periods. Monthly Log Review Checklist
A log that is filled in but never reviewed has half its value. Once a month, take fifteen minutes to scan the binder for:
- Missed entries or blank fields. Any cycle without an operator initial, or a week without a spore test, should be explained.
- Marginal readings. Cycles that ran near the low end of the temperature range on the sterilizer display deserve a maintenance check.
- Chemical indicator failures. If any pack's internal indicator did not change, that cycle's packs should be reprocessed and the sterilizer verified.
- Expiry of supplies. Check the dates on pouches, rolls, tape, and biological indicators that are in active use.
- Training gaps. If a new hire's initials start appearing with frequent blank fields, schedule a refresher on the log routine.
Record the review itself — a dated signature on the last page of the month is enough. That signature is the difference between a binder of forms and a managed quality assurance program.
What an Inspector Looks For in the Log
When a public health officer or provincial regulator reviews your sterilization records, they are not checking whether your handwriting is neat. They are looking for evidence of a functioning system:
- Continuity. Entries for every working day, including the days the clinic was busy and short-staffed. Gaps without explanation are a red flag.
- Weekly spore tests. A biological indicator entry every week per sterilizer, with the result recorded the following day. The CDC's disinfection and sterilization guideline sets weekly biological monitoring as the standard, and regulators expect to see it reflected in the log.

Sterilization Monitoring Beyond the Log
The log records the results; the monitoring products produce them. A complete program uses three complementary layers, and the log is where they all come together:
- Chemical indicators on every pack. Autoclave indicator tape on the outside of every wrapped pack confirms exposure to steam, and a Class 4 indicator inside the pouch confirms temperature and time. What should a sterilization log include?
A complete entry records the date and time, sterilizer identification, cycle type and parameters, load contents, chemical indicator results, biological indicator lot and result, mechanical readings, and the operator's initials. Add notes for any corrective action.
How long do I need to keep sterilization records?
Follow your provincial regulator. Ontario's RCDSO and most provincial bodies expect records to be retained for several years so past cycles can be reconstructed. When in doubt, keep them longer rather than shorter.
Can I keep a digital sterilization log instead of paper?
