An ultrasonic cleaner removes debris from instrument joints, serrations and lumens that manual scrubbing cannot reach. What it cannot do is overcome the wrong solution, a cold bath or a tank loaded past its capacity. Most reprocessing failures traced back to the ultrasonic stage come down to three decisions: which chemistry, how often the bath is changed, and how the machine is verified.

What the Tank Is Doing

The transducer drives the bath at roughly 40 kHz, producing bubbles that implode against instrument surfaces. That cavitation lifts blood, cement and bioburden out of the geometry that brushes miss. It is a cleaning step, not a disinfection step, and it must be followed by rinsing, inspection, packaging and sterilisation.

Water quality matters. Hard water leaves mineral film that shows up later as staining on instruments and cloudy pouches, so most facilities use softened or deionised water in the bath.

Which Solution to Choose

Three chemistries cover most dental and clinic work. Neutral pH solutions suit aluminium and other reactive metals. Enzymatic solutions break down protein and are the common default for general instrumentation. Alkaline solutions handle heavy organic load but must be checked against the manufacturer's instrument instructions, because they can attack anodised finishes.

Temperature and Time

Enzymatic solutions typically perform between 40 °C and 55 °C. Above that range the enzymes denature and the cleaning benefit drops; below it, activity slows and the cycle needs to run longer. Three to ten minutes is a common exposure window, with heavily soiled or complex instruments at the longer end.

Changing the Bath

Solution does not last a week. Change it at least daily, and immediately after any load with visible blood, heavy cement or a known infectious case. A bath that looks clear can still carry enough protein to recontaminate the next load, which is why visual appearance is a poor trigger.

Loading discipline is the last variable. Instruments should sit in a perforated tray or basket, in a single layer where possible, with hinges open and trays not touching the tank walls. A full tank of loosely piled instruments performs worse than a half load arranged properly.

Verification

Ontario's public health reprocessing guidance recommends regular performance testing of the ultrasonic unit, covering both the cleaning efficacy and the energy delivered to the tank. The simple aluminium foil test is the classic quick check: a strip suspended in the bath should perforate evenly across its surface within the cycle. An uneven pattern indicates dead zones, often caused by overloading or insufficient degassing.

Mistakes That Show Up in Audits

  • Instruments stacked so they shadow each other in the tank.
  • Hinged instruments left closed, trapping debris in the box lock.
  • No lid, so aerosols contaminate the surrounding bench.
  • Solution topped up instead of changed.
  • Test results recorded only when a problem is noticed.
