An autoclave only sterilizes the items you load into it correctly. All-plastic pouches rely on a vented seal and appear more often with heat-sensitive items or where a moisture barrier matters.

Wrapped instruments — cassettes and trays in sterilization wrap — come into play when larger sets move between operatories. Wrapped packs behave differently in the chamber: they hold more air, take longer to dry, and need more space between them. Mixing wrapped packs with pouches in one load is possible, but it extends cycle time, so most practices run pouch loads and wrapped loads separately.

Loading patterns: paper-to-paper, plastic-to-plastic

The most practical rule in autoclave loading is also the simplest: arrange pouches so paper faces paper and plastic faces plastic. Steam enters through the paper side and leaves the same way. If a paper side presses against a plastic side, the plastic blocks the steam path and the paper cannot dry properly. Pouches should sit on edge, standing like files in a drawer, rather than stacked flat.

A few patterns worth building into the routine:

  • Stand pouches on edge, paper side facing another paper side.
  • Leave one to two centimetres between pouches so steam can circulate.
  • Keep pouches clear of the chamber walls and the door gasket.
  • Avoid stacking more than a few layers deep; overloading is the most common cause of incomplete sterilization.
  • Place heavier instruments on the lower tray and light pouches above.

Spacing, overloading, and steam penetration

Steam sterilizes by direct contact. It must reach every surface of every instrument, which means the chamber cannot be packed like a suitcase. Overloading is the most frequent cause of failed cycles in small office autoclaves. When pouches sit too tightly, air pockets remain, steam cannot displace them, and the load comes out unsterile even though the cycle display reads complete. A chamber that is about eighty percent full sterilizes more reliably than one that is ninety-five percent full; leave margin for air to escape.

Both treat loading density and steam penetration as process requirements, not suggestions. Order of processing and drying

Process the load in a logical order. Place the biological indicator — or a Class 5 integrator, depending on your monitoring protocol — in the part of the chamber that is hardest for steam to reach, usually the centre of the load or the bottom tray. Load instruments in the order they will be used, so the first pouch out of the chamber is the first one needed at the chair.

Drying deserves as much attention as the sterilization phase. Instruments that come out wet are vulnerable the moment the pouch is opened, and damp paper can wick contamination from the countertop into the pack.
