Medical Waste Terminology: Autoclave vs Incineration vs Chemical

An LTC director asked us to explain one line in her waste contract: "treated by approved method." The hauler meant autoclaving, but she was not sure whether her red bags were incinerated or processed another way — and a resident's family member had asked where the waste actually goes. The terminology is rarely explained in plain language.

Understanding the terms in your waste contract matters: compliance documents reference them, costs differ by method, and not every stream is treated the same way.

Regulated Medical Waste: What It Includes

Regulated medical waste (RMW) carries a risk of infection or injury: items contaminated with blood or body fluids, sharps, laboratory cultures and pathological waste. Rules vary by province, but the principle is consistent — the waste must be contained, transported and treated so it no longer poses a public health risk.

At the facility level, RMW is collected in containers that meet CSA Z316.6, the Canadian standard for medical waste containers, and colour-coded for easy identification — most commonly red bags for biohazardous waste.

Autoclave Treatment

An autoclave treats waste with steam under pressure, typically at 121°C for 30 to 60 minutes, to inactivate microorganisms — the same technology used for surgical instruments. After treatment, waste is shredded or compacted and landfilled as non-infectious.

Autoclaving is the most common method for infectious waste in Canada: effective, with low air emissions, and it costs less than incineration. The limitation: it does not shrink volume much before shredding, and it is unsuitable for cytotoxic or certain chemical wastes.

Incineration

Incineration burns waste at high temperatures — often 850°C or more — reducing it to ash and gases. It destroys pathogens completely and is required for anatomical waste, cytotoxic drugs and controlled substances in many provinces. Modern incinerators include emission controls for dioxins, mercury and particulate matter.

Incineration costs more per kilogram than autoclaving due to capital equipment, fuel and emission controls. Low-volume generators of pathological or cytotoxic waste usually contract a regional facility rather than operating their own unit.

Chemical Treatment

Chemical treatment uses disinfectants — chlorine-based compounds or peracetic acid — to inactivate microorganisms in liquid or semi-liquid waste. It suits laboratory and liquid infectious waste where heat treatment is impractical, requires careful contact-time control and is not approved for sharps, anatomical waste or large solids.

Other Terms You Will See in Contracts

  • Microwave treatment — uses microwave energy to heat and disinfect waste; less common in Canada but present in some regional facilities.
  • Maceration — grinding waste with disinfectant, often used for certain animal or laboratory wastes.
  • Shredding — size reduction after treatment, usually to make the waste unrecognizable before landfill.
  • Irradiation — ionizing radiation treatment for specific waste streams, rarely used at municipal scale.

What the Bag Colour Actually Tells You

Bag colour is a communication system between your facility and the treatment plant. Red signals biohazardous waste for autoclave or incineration; clear or white is used in some provinces for waste that will be macerated or chemically treated; yellow signals cytotoxic waste, which must be incinerated, never autoclaved.

Mixing streams in the wrong bag is a safety and compliance problem. A red bag of cytotoxic waste reaching an autoclave can aerosolize hazardous drugs. Segregation at the point of generation is the first rule of compliant handling — the red biohazard bag is not interchangeable with a general-purpose liner.

Choosing a Treatment Partner and Bag Supply

Ask your waste hauler three questions before renewing: what treatment method is used for each stream, which streams require incineration in your province, and can they provide a certificate of treatment for each pickup. A reputable hauler documents the chain from your dock to the facility.

On the supply side, match your bag inventory to your waste streams. Is autoclaved medical waste still considered medical waste?

After effective treatment, the waste is generally reclassified as non-infectious solid waste and can go to landfill. Provinces require documentation of the treatment, which is why haulers provide certificates of treatment.

Why can't cytotoxic waste be autoclaved?

Heat does not reliably destroy cytotoxic drugs, and autoclaving can aerosolize them. Most provincial rules require incineration of cytotoxic waste. It should be collected in designated yellow or labelled containers, never red bags destined for an autoclave.

What does CSA Z316.6 cover?

CSA Z316.6 is the Canadian standard for medical waste containers, covering leak resistance, puncture resistance, labelling and suitability for the intended treatment method. Containers meeting the standard carry a visible mark or certification.

How do I know which treatment method my hauler uses?

Ask for it in writing and keep the certificate of treatment for each pickup. If your contract only says "approved method," request the specific process — autoclave, incineration, chemical or microwave — for each waste stream you generate.
