Home Visit Bag Technique for Community Nurses: What to Clean Between Visits

Eight visits in a day means eight different kitchens, eight different bathroom counters and eight chances to carry something from one home into the next. The bag is the common object in all eight, which is why bag technique carries more weight in home care than any single piece of equipment inside it.

The bag is a workspace, not a suitcase

Treat the outside of the bag as contaminated and the inside as clean. Set the bag on a barrier — a clean towel, a plastic sheet or the paper liner the bag carries — and never on a bed, a sofa, a kitchen table or the floor. If a client wants the bag on their table, put the barrier down first and wipe the exterior when the visit ends. The clean zone inside holds supplies that touch the patient; anything that has been in the room and used stays in the outer pocket or the waste bag until it is disposed of.

The between-visit reset

Biodegradable waste bags used to contain home visit waste between patients

  • Hand hygiene before touching the bag, using rub carried on the outside of the bag.
  • Wipe the handles, the zipper pulls and the exterior with a compatible disinfectant wipe.
  • Dispose of waste from the visit into a sealed bag before it goes back in the car — never carry loose soft waste between homes.
  • Check the sharps container: below the fill line, closed, and still in its hard case.
  • Restock what the visit consumed so the next home does not start with a shortfall.
  • Replace the barrier cloth or paper liner.
Between-visit step Time Why it matters
Hand rub before and after handling the bag 30 s Breaks the hand-to-bag-to-patient route
Wipe handles and pulls 30 s Removes the highest-touch surfaces
Seal and stow waste 60 s Prevents loose waste travelling in the car
Sharps check and restock 60 s Prevents an overfilled container being carried

What stays in the bag

Keep the contents fixed and counted. Gloves, hand rub, single-use prep pads, gauze, tape, dressings, a clean barrier, a sealed waste bag, a small rigid sharps container, a thermometer with probe covers, and the documentation the visit needs. For hand hygiene between homes, a 60 mL moisturising hand rub (10-pack) clips to the outside of the bag where it is used, and a refill pack keeps it topped up. 4 mil nitrile examination gloves in the clean zone, hypoallergenic paper tape for dressing securement on fragile skin, and a roll of pla biodegradable waste bags for waste covers the routine without overloading the bag.

Sharps and waste inside a home

Never leave a sharps container in a client's home unless the program has an agreement to do so, and never put a loose needle in a household bin. Carry a small rigid container, close and lock it before transport, and dispose of it through the program's biomedical waste route. Soft waste that has contacted body fluids goes into a sealed bag and then into the same biomedical stream; household waste that has not been contaminated does not. Public Health Ontario's infection prevention material and the WHO hand hygiene guidance are the references most community programs cite when they write this routine into policy, and both are worth keeping in the bag in printed form for new staff.

Frequently Asked Questions

Where should a home care bag be placed during a visit?

On a clean barrier, never on a bed, kitchen table, sofa or the floor. The outside of the bag is treated as contaminated and the inside as clean.

How often should the nursing bag be cleaned?

Wipe the exterior handles and pulls between visits, and do a fuller clean at the end of each day, including the internal dividers and the barrier cloth.

Can sharps be left in a client's home?

Only if the program has a written agreement and a compliant container is provided. Otherwise carry a small rigid sharps container and take it back for biomedical waste disposal.

What should never be carried loose in the bag?

Used needles, loose soft waste, and any supply that has been in the patient zone. Keep used items in the outer pocket or a sealed waste bag until disposal.

How should specimens be carried between homes?

In a leakproof, rigid secondary container kept apart from clean supplies, with the requisition bagged separately; the vehicle seat is not a transport surface.
