ISO 15223-1 Symbols on Medical Packaging: What They Mean
Nothing on a supply shelf carries as much meaning per square centimetre as a printed symbol. A small circle with a two tells you the item is single use. A crossed-out sun tells you where not to store the carton. A sterile symbol tells you which barrier has been validated.
Most clinic staff were never formally taught the set. They learn it the way everyone learns it, from a colleague, which is how a storeroom ends up with sterile stock against a south-facing window.
What does ISO 15223-1 cover?
ISO 15223-1 is the standard that defines the symbols used on medical device labelling, together with the accompanying information a maker has to supply. It exists so that a symbol means the same thing on a mask carton, a pouch box and an instrument insert, whoever manufactured them.

The standard does not decide whether a product is good. It decides whether the symbol on the carton can be read the same way in Halifax and in Hamilton.
Which symbols matter most in a clinic storeroom?
| Symbol meaning | What it tells you | What goes wrong when it is ignored |
|---|---|---|
| Single use | One patient or procedure, no reprocessing | Items are soaked and reused with no validated method behind it |
| Sterile barrier | Contents were sterilized inside this pack | Damaged or wet packs are shelved as sterile |
| Do not resterilize | The maker did not validate a second cycle | Pouches are run again and the barrier is compromised |
| Keep away from sunlight | Material or reagent is light sensitive | Cartons are stored on a window shelf |
| Store between temperature limits | Range the product was validated to | Cases sit in an unheated loading bay over winter |
| Consult instructions for use | A condition applies that will not fit on the label | Staff never open the insert |

Where do these symbols connect to clinic records?
Storage and sterility symbols feed straight into the records an inspector asks for. If a product has a storage range, the room it is kept in is part of the control. If a pouch is marked as a single sterile barrier, the condition of that pouch at the point of use is part of the check.
This is why the practical habit is to read the carton once when a product is first stocked, write the storage rule onto the shelf label, and stop relying on memory.
How should a buyer use symbols when comparing suppliers?
Ask three questions. Does the carton carry legible symbols and bilingual text. Does the supplier provide the instructions for use on request. And can they confirm the sterilisation method behind any product marked sterile.
Then match the product to the room. Single-use procedure masks belong in a drawer, not on a windowsill. Nitrile exam gloves need a shelf away from heat and sunlight. Sterilization pouches need a dry, dust-free box that keeps weight off the seals.
To standardise labelling and storage rules across several rooms, request a bulk quote and we will send the packaging information with the price. Where sterilization loads are released on a monitored cycle, the 5-pack biological indicator trial adds an independent spore result to the paperwork, at CA $12.99 with shipping included.

Frequently Asked Questions
Why are medical products labelled with symbols instead of words?
Symbols carry the same meaning across languages and markets, so one printed package can ship to several countries. The trade-off is that the meaning lives in a standard your staff may never have been trained on.
What does the single-use symbol mean in practice?
It means the product is intended for one patient or one procedure and is not designed to be cleaned and reused. Reprocessing a single-use item moves it outside its validated instructions and puts the facility, not the maker, on the hook.
Does an expiry date on packaging always mean the product stops working?
Not in the same way for every product. For sterile items the date is tied to the sterile barrier, for chemical indicators to the reagent, and for some consumables it reflects a material change. The date on the package is the one to follow.
Do symbols on an imported product have to appear in French in Canada?
Federal rules for medical devices require both official languages on the label, and Quebec adds its own expectations for consumer-facing text. If your carton only carries English, ask the supplier to confirm the labelling before you standardise on it.
