ISO 2859-1: AQL Sampling Plans for Incoming Supply Inspection

Most clinics accept a delivery by counting boxes and checking the invoice, then discover a problem two weeks later when a glove tears during a procedure. ISO 2859-1 is the standard that turns incoming inspection into a decision rule instead of an impression.

It answers a narrow but useful question: how many items should come out of the carton, and how many defects mean the lot should go back?

What is ISO 2859-1?

It is the part of the ISO 2859 series that sets out sampling plans indexed by acceptance quality limit, usually shortened to AQL. The plans are attribute-based, which means each item inspected is judged pass or fail on a defined characteristic rather than measured on a scale.

The standard also sets inspection levels. General levels are used for routine acceptance, and special levels are available when a smaller sample is needed, for example during a first delivery from a new supplier.

How does lot size become sample size?

The code letter also carries the acceptance and rejection numbers at the agreed limit. A plan that accepts up to a defined number of defects and rejects above it is what makes the result repeatable between two people inspecting the same lot.

How would a clinic use this in practice?

Pick the characteristic that matters for the product. For nitrile examination gloves it is usually a visual and tactile check for holes and weak points, which is how the glove specifications approach barrier quality. For procedure masks it is the ear loop attachment, nose wire and ply separation.

Then draw the sample at random from across the shipment rather than from the upper layer of the first carton, and record what you found. A one-page log with the date, lot number, sample size and defect count is enough to raise a supplier conversation with evidence.

Where does this fit alongside the supplier's own testing?

Incoming inspection is not a replacement for the supplier's quality system. The manufacturer tests to specifications such as the glove or mask standard, and isolation gowns, masks and gloves should arrive with the documentation that supports those claims. Your sampling tells you whether this particular shipment looks like the ones that were tested.

What does AQL stand for?

Acceptance quality limit. It is the quality level that is considered tolerable as a process average. A supplier and buyer agree a limit, and the sampling plan is then built so that lots at or better than that level will usually be accepted.

How is the sample size chosen?

Two inputs are needed: the lot size and the inspection level. The lot size gives a code letter, and the code letter plus the agreed acceptance quality limit gives the number of items to examine and the number of defects that will still allow acceptance.

Do I have to inspect every box?

No. That is the point of a sampling plan, and examining a representative sample costs less and is often more consistent than a quick look at everything. The plan tells you how many items to draw and how to draw them so the sample represents the lot.

What happens if a lot fails?

The plan produces a decision, not a diagnosis. A failed lot is withheld and the supplier is asked to sort, replace or credit it, and the reason is recorded so the same failure can be recognised in the next shipment.
