An inconclusive autoclave tape strip does not tell you the instruments failed to sterilize, and it does not tell you they are sterile. Tape is a Class 1 process indicator, so it confirms only that a package went through a cycle — not that time, temperature and saturated steam were enough to kill spores. A faded or uneven strip means hold the load and check the cycle.

What a faded strip is actually telling you
Tape changes colour because a printed chemical ink reacts to heat, time and moisture at the outside of the package. That is the whole job, and it has never been a kill test.
An external indicator reports on the outside of the pack only. An internal chemical indicator sits inside the load, where steam has to travel further, which is why dark tape and a pale internal strip turn up in the same run.

These three tools answer different questions, and treating them as one is where clinics get caught. Tape confirms the package went through a cycle; an integration indicator responds to the critical variables together.

Why a strip can look faded, blotchy or uneven
Most inconclusive strips trace back to the load or the cycle rather than to the tape. The ink needs heat, time and saturated steam across its whole printed surface.
Packs stacked tight leave cool spots where air and condensate collect, wet steam gives a patchy change, and rolls kept in a hot, humid cupboard lose sensitivity long before they run out.
When the strip looks inconclusive: hold, re-process, root-cause
The response should be procedural and identical every time, so it does not depend on who is on shift. Note the load number, date and sterilizer ID in the log as soon as the problem is seen.
- Quarantine the load. Label the packs unverified and store them away from the treatment area.
- Re-process. Re-run the full cycle with a fresh strip and release only if it changes evenly.
- Root-cause. Review loading, water quality, cycle time and the tape roll, then record the fix.
