- Anatomical waste — tissues, organs, body parts
- Non-anatomical waste — blood and bodily fluids in quantities exceeding 100 mL per container, contaminated dressings, disposables from isolation rooms with known infectious diseases
- Sharps waste — needles, syringes, lancets, scalpel blades, broken glass contaminated with biohazards
- Microbiological waste — laboratory cultures, stocks of infectious agents
- Pathological waste — biopsy specimens, surgical specimens
Most Ontario clinics generate primarily sharps waste and non-anatomical biomedical waste (contaminated gloves, dressings, tubing). Understanding which category each waste item falls into determines how it must be packaged, labelled, and disposed of.

Segregation Requirements
Proper segregation at the point of generation is the most important compliance step. Ontario’s colour-coded system aligns with CSA Standard Z317.10-21:
- Yellow bags or rigid containers — biomedical waste for incineration (anatomical, pathological, microbiological)
- Red bags or rigid containers — biomedical waste for autoclave/steam sterilization (non-anatomical waste such as contaminated gowns, gloves, dressings)
- Sharps containers — puncture-resistant, leak-proof, rigid containers, labelled with the biohazard symbol, for all sharps waste
- Clear/black bags — general waste (non-hazardous, non-infectious)
Never place biomedical waste in clear garbage bags destined for municipal landfills. Fines under the EPA can reach $50,000 per day for individuals and $100,000 per day for corporations.
Packaging and Labelling Standards
All biomedical waste containers must meet these Ontario requirements:
- Containers must be leak-proof and tear-resistant — single-use bags for non-sharps biomedical waste must be at least 0.076 mm (3 mil) thick
- Sharps containers must be puncture-resistant, rigid, and closeable. Never fill past the manufacturer’s fill line (typically three-quarters full)
- Each container must be labelled with the biohazard symbol, the words “Biomedical Waste” or “Déchets Biomédicaux,” the generator’s name and address, and the date of waste pickup
- Outer secondary containment is required for transport — biomedical waste bags must be placed inside rigid, leak-proof carts or bins
Storage Requirements for Clinics
Ontario regulations limit storage times for biomedical waste:
- Storage time: Biomedical waste must not be stored on-site for more than 7 days in warm weather (above 0°C) without refrigeration. Refrigerated storage (4°C or below) may extend this period
- Storage area: Must be secure, locked, ventilated, and clearly marked with biohazard signage. Access restricted to authorized personnel only
- No compaction: Biomedical waste must not be compacted before treatment
Transportation and Disposal
Clinics must use a licensed biomedical waste carrier for off-site transport. Under O. Reg. 106/21 amendments to Regulation 347, generators must:
- Register as a biomedical waste generator with the MECP if generating more than 50 kg/month (most clinics meet this threshold)
- Complete a Waste Manifest (EPA Part V) for each waste shipment — this tracks waste from generation through treatment and final disposal
- Retain manifests for a minimum of 2 years
Treatment methods permitted in Ontario include autoclaving (steam sterilization), incineration, and chemical treatment. Treated biomedical waste that passes validation testing can be disposed of at approved municipal landfills.

Common Compliance Gaps We See in Ontario Clinics
After helping dozens of clinics review their waste management programs, here are the most frequent oversights:
Overfilling sharps containers. Ontario regulations require containers to be closed when waste reaches the fill line. An overflowing sharps container is one of the most common MOE inspection violations.
Incorrect colour coding. Red bags in clear outer containers, or biomedical waste mixed with recycling — both trigger fines. Train every staff member on the colour system.
Missing manifests. If your waste carrier doesn’t provide a manifest for every pickup, you are non-compliant. Request manifests proactively and file them by month.
No generator registration. Many small clinics assume they are exempt. If your clinic generates over 50 kg of biomedical waste per month (roughly two full red bag liners per week), you must register with the MECP.
